Information you choose to provide
The consultation form transmits the contact, organization, facility, project, survey, consent, and optional file information you provide to Life Safety Express so the request can be evaluated and answered. The submission is delivered through the website host’s email system and may be retained in business email records for follow-up and recordkeeping.
Do not enter patient information, protected health information, account credentials, or other sensitive clinical data in a public website form or file field.
Technical information
The hosting provider may process basic request information such as an IP address, browser details, requested pages, timestamps, and security events to deliver and protect the website. The form uses a temporary one-way hash derived from the requester’s IP address to enforce an hourly submission limit; the form handler does not create a website account or permanent upload library.
Cookies, analytics, and third parties
This application does not currently include advertising cookies, behavior analytics, scheduling software, or marketing pixels. The website host and email provider process consultation submissions for delivery. If additional analytics, consent, scheduling, or form providers are enabled, this notice and the cookie policy will be updated.
Your choices
You may choose not to use the consultation form. Privacy questions and requests to access, correct, or delete submitted information may be directed to larrybarlow@lifesafetyexpress.com. Requests remain subject to applicable law and legitimate retention obligations.
