Preserve the issued deficiency statement, survey tag or requirement reference, provider type, building, location, date, observed facts, and response deadline. Separate the surveyor's statement from the facility's technical analysis and avoid paraphrasing away important scope or evidence. Sustained evidence should show that the facility changed the condition and the system that allowed it, rather than producing a one-time package that stops being used after the response is submitted.5678
